The Recalibration Pyramid

My treatment approach follows a pyramid structure. The base focuses on improving range of motion and strength, and as we progress, the movements become more complex. Each level of the pyramid relies on the foundation established in the previous one.

Patients are not advanced to the next phase until the goals of the current phase have been fully addressed and achieved. Only after that is the transition made to the next stage of treatment.

For example, if a patient comes to me with knee pain and an inability to run, I would follow this approach:
  1. Address Range of Motion: Start by resolving any lower body range of motion deficits.
  2. Isolate Strengthening: Focus on strengthening individual muscle groups (e.g., hamstring curls, calf raises, lunges) to correct imbalances.
  3. Introduce Squats: Add squats once strength is balanced. Squats require both feet on the ground and should only be introduced when the knee is stable and strong enough to prevent compensations.
  4. Progress Squats: Gradually increase squat resistance to body weight or near-body weight levels as strength improves.
  5. Add Plyometrics: Incorporate plyometric movements like jumping and hopping to build explosive power.
  6. Introduce Running: Begin interval running if pain-free, progressing toward continuous running.

Focuses on higher-level performance abilities, such as running and sprinting.

Tests the ability to perform repetitive, forward-directed movements like multiple hops.

Evaluates the client’s ability to generate power, such as performing jumps.

This level measures overall strength and capacity, including the ability to perform one-rep or ten-rep squats.

At this stage, the focus is on assessing tolerance for a full session and identifying any joints that are not moving through their full range of motion.

This foundational level determines whether a client is a candidate for physical therapy or needs a referral to another specialist.
